Submit a Claim for Damages

Step 1.Complete form

Claims for personal injury, death, or property damage must be filed within  6 months (See Government Code section 911.2); all other claims within 1 year (Government Code section 911.2).

Filing a false claim is a felony. (Penal Code §72)

Step 2.Sign and submit 

Once you sign the form, you can either mail it to us or bring it to us in person at:

Step 3.We'll get back to you

Once we receive your claim, we'll assign it a claim number, and a claims adjuster will investigate it. We'll process your claim within 4 to 6 weeks and notify you of the outcome.
