BASICS for NEW PASSPORT
- Proof of U. S. Citizenship (certified birth certificate, original naturalization paper)
Proof of identity (driver's license, or other government-issued ID)
One recent, color photograph (2" x 2")
NOTE: Minors must appear in person; both parents or legal guardians must also appear and provide identification; if one parent is unable to appear a notarized statement of consent must be submitted.

BASICS for PASSPORT RENEWAL BY MAIL
Renewals can be made by mail, IF ALL of the following criteria are met:
Can submit most recent passport.
Was at least 16 years old when most recent was issued.
Was issued most recent passport less than 15 years ago.
Use the same name OR have had name changed by marriage or court order and can submit proper documentation.
